Job Title: Case Manager/Discharge Planner Profession: Registered Nurse Specialty: Case Manager Duration: 13 weeks Shift: Day Hours per Shift: 5x8 (08:00 - 16:30) Experience: Minimum 2 years in discharge planning, including admission criteria review, benefits eligibility, care coordination, conducting needs assessments, and ordering DME. Prior charge experience preferred. Completion of at least 1 travel assignment. License: DC RN Licensure (Will accept pending with proof of application) Certifications: BLS Must-Have: Will accept RN Case Manager/Discharge Planner or Licensed Social Worker (must be eligible for LMSW/LGSW/LCSW). Description: The Case Manager/Discharge Planner will be responsible for discharge planning within the facility. The ideal candidate will have experience with home health, hospice placement, LTAC, nursing home placement, and rehab placement. Acute hospital experience is also required. Regulatory skills in CMS, HIPAA, and workers compensation are preferred. The unit focuses on patients with diagnoses related to geriatrics, ICU, heart and vascular, general surgery, and general medicine. Charting will be done using Cerner. A professional appearance is required, with a business casual uniform. All employees must have the flu vaccine without exemptions.
