Job Description:
At Agape Care Group, we hold the belief that spiritual
well-being is an essential part of comprehensive hospice and
palliative care. As a Spiritual Resource Coordinator, you will play
a crucial role in facilitating spiritual support tailored to
individual beliefs and preferences of our patients and their
families. Your commitment will help enhance their comfort and
dignity during the most poignant moments of life. Conduct spiritual
assessments to identify patient and family needs and preferences.
Develop and implement personalized spiritual care plans in
collaboration with a multidisciplinary team. Provide direct
spiritual support and counseling to patients and families.
Coordinate with local clergy and spiritual counselors to arrange
customized services and support. Educate and lead staff on the
importance of spirituality in patient care and holistic health.
Organize and facilitate grief support groups and memorial services.
Maintain patient confidentiality and document all care provided in
patient records. Strong interpersonal and communication skills
Empathy and sensitivity to diverse spiritual, cultural, and
personal beliefs Ability to work collaboratively in a
multidisciplinary team Excellent organizing and leadership skills
Comfortable with public speaking and conducting group sessions

401k Board Certified Chaplain (BCC) by the Association of
Professional Chaplains Certified by the National Association of
Catholic Chaplains (NACC) or equivalent credential
