Agape Care Group is seeking a compassionate and dedicated Licensed Medical Social Worker to join our team, providing exceptional social work services in hospice and palliative care environments across the Southeast. Our ideal candidate is someone who is genuinely passionate about end-of-life care and has a proven track record of improving patient and family outcomes through comprehensive psychosocial support. Assess patients’ and families' psychosocial needs, developing and implementing tailored care plans. Provide counseling and support to patients and their families, helping them navigate emotional, social, and financial challenges. Collaborate with an interdisciplinary team to ensure an integrated approach to care, maximizing comfort and quality of life. Facilitate community resources planning and intervention to ensure patients receive comprehensive support and services. Support the patient admission process and contribute to patient education and advocacy initiatives. Document all patient interactions and interventions accurately and comprehensively in patient care records. Strong interpersonal, counseling, and advocacy skills. Ability to work effectively within an interdisciplinary team. Excellent organizational skills and the ability to handle multiple priorities effectively. Proficient in the use of electronic health records.
