SUDP Hourly Range:$23.08 - $33.65 Hourly ($48,000 - $70,000

Who should apply?
- Substance Use Disorder Professional Certification with the
Department of Health.
- High School Diploma or equivalent required.
- Associates degree in Social Services, counseling is
preferred.
- Positive, solutions oriented clinicians.
- Therapists skilled at engaging and motivating youth to achieve
participation in services.
- Ability to meet with clients and colleagues in homes and
community locations as needed.
- Valid driver's license & current automobile insurance, reliable
transportation with an acceptable driving record.Some of what you
will do:
- Assess and identify the treatment services individual in
service individual in services should receive are based on severity
of substance use diagnosis, stage of change and immediate needs,
and recommend treatment objectives for the individual in service
individual in service's service plan.
- Will be on-call as needed.
- Contribute to the program's ongoing assessment of participant's
SUD treatment needs and mental illness symptoms and participant's
response to treatment. Shares observations and concerns with team
members for consultation and direction and takes appropriate
action.
- Develop comprehensive service plans that include built in
review and evaluation points, and modifies the service plan in
collaboration with the individual in services support system.
- Document participant progress to maintain a permanent record of
activity according to the agency's established methods and
procedures.
- Provide individual relapse prevention, harm reduction and
substance abuse counseling based on motivational interviewing and
cognitive behavioral responses to mental illness and substance
abuse.
- Work effectively with the individual in services support
system, education and vocational training providers, and self-help
groups providing services to individuals in services. Connect
individuals in services to self-help recovery groups.
- Assess and provide supportive services to address needs for
problem-solving, wellness self-management, housing, income support,
education and vocational training, social support, employment, and
primary care.
- Evaluate, on a continuing basis, services and expected
outcomes. Be an advocate and liaison for individuals in services in
securing emergency detox and rehabilitation care, and other
community supports that promote recovery.
- Meets expected productivity standards and completes the
necessary financial/billing information for the organization to
