Health experience or 1 full year of Hospice experience License:
Must have a valid nursing license in hand Certifications: CPR, BLS
Must-Have: - Recent specialty experience in Home Health or Hospice
- Proficiency in HomeCare HomeBase EMR and OASIS Description:
Provides coordinates and directs the provision of home nursing care
according to physician orders based on agency policies and
procedures through the competent application of the nursing
process. Responsible to the Clinical Manager. Directly supervises
and evaluates Home Health Aides and Licensed Practical
Nurses/Licensed Vocational Nurses. Home visits and periodic office
meetings are required. Reliable transportation and a current
driver's license along with agency-required liability insurance are
necessary. Average number of patients per day is between 5 to 8.
Contractor will be assigned between 5 to 10 patients per day that
equate to roughly 6 to 8 points. No guaranteed hours are offered.
If there are instances where the contractor cannot meet their
financial needs due to low patient census, it is encouraged to
reach out to the appropriate personnel to discuss floating to a
nearby clinic to pick up hours. Types of cases include Start of
Care, Wound Care, Follow-up Visits, etc. Must be willing to travel
up to 75 miles from the home clinic. Mileage is reimbursed at the
IRS standard rate. Travelers are compensated for mileage from
patient home to patient home. The commute to and from the first and
last patient is not billable. Local candidates are accepted at the
same rate. Completion of all documentation must occur within 24
hours of the appointment with the patient. Delayed and incomplete
documentation that impacts billing and follow-up patient care may
result in cancellation. Weekly productivity expectations are 30
points, which correlates to roughly 40 hours per week. All overtime
must be pre-approved. If insufficient patients are received each
week to support financial needs, there will be the opportunity to
float to another nearby clinic to pick up additional patients. 5
